Care Management Pilot to Enhance Service Delivery


In a symbolic moment to celebrate funding that was launched with the Home State Health managed care organization's pilot program, CommCARE President and CEO Terry Trafton delivers the first funding check to Tom Petrizzo, CEO of Tri-County Mental Health Services (see photo above).     The pilot program will focus intensely on 20 individuals. The goal is to find ways to bring higher levels of care to at-risk individuals who need special help accessing services for illnesses that include paranoid schizophrenia, bipolar disorder, major depressive disorder, post-traumatic stress disorder, and more. 

Trafton says, "Tri-County, along with eleven other agencies that are part of CommCARE's Independent Practitioners Association, have been working hard with staff from CommCARE and MO Coalition for Community Behavioral Healthcare. Our collaborative goal is to help at-risk Medicaid members connect to services that will help them attain better health outcomes. CommCARE is delighted that Tri-County is an important part of our project. Together, we can make sure people can live healthier lives.”
